Pharmacological action

Tetrizoline is a sympathomimetic drug that stimulates α-adrenergic receptors of the sympathetic nervous system, but has no or weak effect on β-adrenergic receptors. As a sympathomimetic amine, tetrizoline has a vasoconstrictor effect and reduces tissue swelling.

The effect begins 60 seconds after instillation and lasts 4–8 hours.

Pharmacokinetics

When applied topically, systemic absorption is possible in patients with damage to the mucous membrane and epithelium. Detailed pharmacokinetic studies have not been conducted following topical application of eye drops.

Indications

  • adults and children over 2 years of age to relieve swelling and hyperemia of the conjunctiva (redness of the eyes) caused by exposure to chemical and physical factors (smoke, wind, dust, chlorinated water, light, cosmetics, contact lenses), as well as those arising from allergic reactions such as like hay fever.

Contraindications

  • angle-closure glaucoma;
  • endothelial-epithelial dystrophy of the cornea;
  • children under 2 years of age;
  • hypersensitivity to the components of the drug.

With caution the drug is prescribed to patients with severe cardiovascular diseases (CHD), pheochromocytoma, prostatic hyperplasia, hyperthyroidism, diabetes mellitus, porphyria, dry rhinitis, keratoconjunctivitis sicca, glaucoma and in patients receiving MAO inhibitors or other drugs that can increase blood pressure.

Dosage

The drug is used topically.

Adults and children over 6 years old Prescribe 1 or 2 drops into the affected eye 2-3 times a day.

Instructions for using the dropper bottle

The medicinal product is supplied in packaging that is protected from accidental opening by children.

When using the bottle for the first time, you must remove the tamper evident tape from the cap.

Press the top of the bottle cap while turning it counterclockwise. Remove the cap from the dropper bottle and invert the bottle.

Do not touch the tip of the bottle to any surfaces.

Open the ampoule by unscrewing the top, unfilled part of the ampoule.

Overdose

However, if the drug accidentally gets into the gastrointestinal tract (swallowed), the following are possible: symptoms overdose: pupil dilation, nausea, cyanosis, fever, convulsions, tachycardia, arrhythmia, cardiac arrest, arterial hypotension, pulmonary edema, respiratory depression, including apnea (respiratory arrest), hypothermia, depression of central nervous system function, sedation, lethargy, stupor, drooling, development of drowsiness, coma.

The risk of developing overdose symptoms due to the systemic effects of the drug is high in neonates and young children, especially if swallowed.

Treatment: a specific antidote is unknown. If it enters the gastrointestinal tract, gastric lavage, oxygen inhalation, antipyretics and anticonvulsants are prescribed. To reduce blood pressure, phentolamine is used slowly intravenously at 5 mg in saline solution or orally at 100 mg. Vasopressor agents are contraindicated in patients with low blood pressure.

If any of the overdose symptoms described above occur, the patient should immediately consult a doctor.

Drug interactions

The drug Visin Classic should not be used simultaneously with MAO inhibitors such as tranylcypromine, tricyclic antidepressants, or drugs that increase blood pressure. Combination therapy with these groups of drugs can lead to increased vasoconstrictor effects and increased blood pressure.

Special instructions

Visin Classic should be used with caution in elderly patients, in patients with aneurysms, hypertension and/or coronary artery disease, as well as in patients with or hyperthyroidism.

The drug in dropper bottles contains benzalkonium chloride, which can cause eye irritation and discoloration of soft contact lenses. Therefore, before instilling the drug, it is necessary to remove contact lenses and install them after 15 minutes. It is necessary to avoid direct contact of the drug with soft contact lenses due to possible disruption of their transparency.

It is possible to develop reactive hyperemia of the conjunctiva and nasal mucosa (medicinal rhinitis) if the instructions for use are not followed.
