Pharmacological action: analgesic, anti-inflammatory, antiseptic.

Pharmacological properties

Benzidamine stabilizes cell membranes, inhibits the formation of prostaglandins, and inhibits the enzyme cyclooxygenase. The antiseptic activity of benzydamine is especially pronounced against a number of microorganisms, including Gardnerella vaginalis. Benzidamine penetrates easily through mucous membranes and quickly reaches sites of inflammation. Benzydamine is excreted by the intestines and kidneys in the form of metabolites.

Indications

Inflammatory processes in the throat and oral cavity: pharyngitis, post-tonsillectomy conditions, gingivitis, aphthous ulcers, tonsillitis, stomatitis; inflammation of the oral mucosa after chemotherapy, radiation therapy and surgery; thrombophlebitis and phlebitis of the superficial veins of the lower extremities (as part of combined treatment), condition after phlebectomy and sclerosis of veins, postphlebitis syndromes; insufficiency of the leg veins, including during pregnancy; nonspecific vaginitis (occurring after radiation therapy, before and after operations in this area, the use of intrauterine pessaries), inflammatory processes in the os of the uterus, trichomonas and fungal infections, estrogen treatment in old age with an atrophied vagina.

Method of administration of benzydamine and dose

The mechanism of action of the drug is associated with the stabilization of cell membranes and inhibition of prostaglandin synthesis.

Benzidamine has an antibacterial and specific antimicrobial effect due to rapid penetration through the membranes of microorganisms with subsequent damage to cellular structures, disruption of metabolic processes and cell lysosomes.

Has an antimicrobial effect against Candidaalbicans. Causes structural modifications of the cell wall of fungi and their metabolic chains, thus preventing their reproduction, which was the basis for the use of benzydamine for inflammatory processes in the oral cavity, including infectious etiology.

Pharmacokinetics:

When applied topically, the drug is well absorbed through the mucous membranes and penetrates into inflamed tissues; it is found in the blood plasma in quantities insufficient to produce systemic effects.

Excretion of the drug occurs mainly by the kidneys, in the form of inactive metabolites or conjugation products.

Benzydamine hydrochloride - this name is known only to a narrow circle of specialists. Patients know it as Tantum Verde. Already on the second day of its use, pain, burning, and discomfort in the throat decrease.

Operating principle

To relieve the inflammatory reaction, drugs from the group of non-steroidal anti-inflammatory drugs are prescribed.

Unlike other drugs with similar effects (analgesics, anesthetics, antipyretics), they have several advantages:

  • They act simultaneously on pain, swelling, and inflammation.
  • They have a wide range of applications.
  • Effective for any pathologies.
  • They have a minimal list of contraindications.
  • Do not inhibit the functioning of the central and peripheral nervous system.
  • Do not accumulate in the body.
  • Convenient to use, as they are produced in any dosage forms.

Benzydamine hydrochloride belongs to the group of indazoles and, in addition to anti-inflammatory and analgesic properties, acts as an antiseptic with specific antimicrobial properties.

The main component benzydamine hydrochloride has several distinctive qualities:

  • Has a high absorption rate.
  • Quickly penetrates epithelial structures.
  • Accumulating in the required concentration, it acts effectively at the site of inflammation.
  • Local use ensures a low concentration of the substance in the blood, and this, in turn, makes it possible to avoid systemic effects on the body.
  • Due to the ability to quickly penetrate cell membranes, it disrupts the metabolism of 110 types of pathogenic microorganisms and inhibits their reproduction.
  • Has fungicidal properties (effective on 20 strains of fungi).
  • It inhibits pain receptors locally, and at the same time affects the central parts of the brain responsible for pain sensitivity.
  • It exhibits properties within 30 seconds and retains local anesthesia for about 2 hours.

And, most importantly, it gives a therapeutic effect on the first day of treatment.

There are now about 40 original patented non-steroidal anti-inflammatory drugs on the modern pharmaceutical market. But only one of them contains beznidamine hydrochloride. This is Tantum Verde - a product of the Italian pharmaceutical concern ANGELINI FRANCESCO A.C.R.A.F. S.p.A."

Application

All dosage forms of the drug are used topically, after meals, for otolaryngological and dental pathologies:

  1. Lozenges (one unit contains 3 mg of active ingredient).
  2. Solution for local irrigation and rinsing (1 ml – 1.5 mg) in a 120 ml bottle.
  3. Spray (1 dose 2.5 mg) in a 30 ml bottle.

Dosage forms are prescribed for sore throat, pharyngitis, laryngitis, tonsillitis and candidiasis. In dentistry they are used for periodontal disease and sialadenitis.

For the treatment of infectious diseases and inflammatory processes of the throat, larynx and oral cavity of various etiologies, it is recommended to use a tablet form of the drug for resorption every 6 hours.

In case of severe pain, rinsing or irrigation every one and a half to two hours is recommended.

A metered spray is prescribed in one dose (one injection) 4 to 6 times a day.

For the treatment of children, the calculation of the daily dosage of the spray (according to the instructions) is carried out according to the formula taking into account body weight.

Tantum Verde does not contain antibiotic substances or dangerous auxiliary components, therefore the spray is actively used in pediatrics to treat children from 3 years of age.

Advantages of the form:

  • Overdose of an aerosol and ingestion of the drug by a child are excluded.
  • Absorption occurs gradually.
  • Convenient to use at home.

Benzydamine hydrochloride is also included in the Tantum gel, which is indicated in the complex treatment of venous insufficiency. This is prescribed to patients with swelling and a chronic feeling of heaviness in the lower extremities, with nighttime muscle cramps, pain, phlebitis and thrombophlebitis. Gel Tantum is allowed to be used during pregnancy.

It is used externally, locally in the area of projection of pain or swelling, by rubbing until completely absorbed four times a day.

Concern, based on benzydamine hydrochloride, produces the drug Tantum rose in the form of a powder for preparing a solution (1 sachet per 0.5 liter of water). Used in gynecological practice in the complex treatment of bacterial vaginosis, nonspecific and specific vulvovaginitis, cervicovaginitis.

For preventive purposes, it is prescribed before and after surgical interventions, as well as for hygienic procedures in the postpartum period.

Advantages

All dosage forms containing benzydamine hydrochloride meet the basic requirements for local drugs used to treat acute and chronic pathologies of the oropharynx.

Advantages:

  • They have a wide spectrum of antimicrobial action.
  • They do not cause a systemic toxic reaction.
  • Does not irritate mucous membranes.
  • Hypoallergenic.
  • They can be used as single drugs and as part of combination therapy.

If other representatives of the group of non-steroidal anti-inflammatory drugs are based on weak acids, then benzydamine is a weak base. Thanks to this, the substance penetrates well into the inflammatory focus and accumulates in the required therapeutic concentration.

Due to the antimicrobial and antimycotic properties of the drugs, plaque disappears, and the mucous surfaces of the oropharynx are sanitized in the shortest possible time.

But, despite the fact that the drug is effective and safe for all age groups, only a specialist can make a rational choice in its favor and select an adequate treatment regimen.
